Your first visit to our Milton dental office includes a complete review of your dental needs, health history, and oral health, followed by a discussion of your diagnosis and treatment options.

Take digital X-rays or radiographs (if required)

What To Bring To Your Appointment
To help us process claims efficiently and provide the best care possible, please bring the following to your first appointment:
Your completed Patient History Form
Up-to-date insurance information (insurer name, policy/group number, ID number)
Recent dental X-rays (taken within the last 6 months, if available)
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Save Time — Complete Your Forms
To ensure all your information is current and accurate, we ask new patients to complete a Patient History Form upon arrival or ahead of time. With your permission, we may also request records from your previous dental office.
Insurance & Financial Arrangements
We believe in providing high-quality care on a sound financial basis. Prior to treatment, all fees and financial arrangements will be clearly discussed with you.
